• Acute care GP referral hotline – medical patients only
  For all patients requiring general medicine assessment and/or admission and all potential medical Same Day Emergency Care (SDEC) referrals not suitable for Hospital at Home.
  
  An urgent and emergency care coordinator will triage patients to identify the most appropriate place for them to be assessed and treated and will allocate to SDEC, Medical Assessment Unit or ED depending on the severity.

• Frailty hotline
  For patients 75 years and over or clinical frailty score of 5 or more – urgent medical assessment within 24–48 hours of referral for patients who have complex needs that are deteriorating, and are at high risk of admission. For review in frailty hot clinic within Same Day Emergency Care area, Lister Hospital.

  The Maternity Triage Service is for women who are 20+ weeks gestation with an obstetric concern, or a concern that will directly affect the pregnancy. Acute medical concerns such as chest pain or SOB should be directed to ED
