Consultant and GP Liaison Committee
What is the consultant and GP liaison committee?
The consultant and GP liaison committee brings together hospital consultants with local GPs to identify and resolve any clinical interface issues between secondary and primary care, contributing to maintenance of strong working relationships and open communication between Trust consultants and local GPs.
Some of the responsibilities and duties of the committee are:
- to discuss any issues that arise between the Trust and primary care services that affect the effective integration of patient care services. The focus is the resolution of any issues that may hamper day-to-day clinical interaction
- to focus on influencing and shaping practical issues to improve local clinical pathways within the integrated care system
- to assess and comment on new pathways for integration of patient care between primary and secondary care services issued by the hospital and community trusts
- to discuss ways of reducing workload between hospital and primary care clinicians
The committee is co-chaired by Adam Frosh, consultant ENT surgeon, East and North Hertfordshire NHS Trust and Prag Moodley, partner member – primary care, Hertfordshire and West Essex Integrated Care Board (HWE ICB).
Who is invited to attend the committee?
PCN clinical directors and any East and North Hertfordshire GPs are invited to attend the committee. If you do not already have an invite and would like to attend, please email our GP Liaison team.

Shared Care Record – Hertfordshire and West Essex
What is Shared Care Record?
A shared care record is a safe and secure way of bringing separate records from different health and care organisations together digitally in one place. The Shared Care Record helps staff to make the best decisions by having a more joined-up picture of a patient’s care. Information is available at the point of care throughout a patient’s journey.
Who can access Shared Care Record?
All practice staff who have access to patient records in SystmOne or EMIS can automatically open the Shared Care Record for any patient where direct care is being provided. The Shared Care Record is not the full record for an individual but brings together a valuable summary of information meaning you no longer have to chase departments for simple queries.
What can I see for East and North Hertfordshire NHS Trust?
Information available in real-time includes:
– upcoming appointments
– admissions
– pathology results (excluding cellular pathology)
– endoscopy and radiology reports
– inpatient and ED discharge summaries.

What is Share for Care?
Share for Care is the confidential electronic record sharing framework that is being rolled out across Bedfordshire, Luton, and Milton Keynes (BLMK).
How to access Share for Care
There are currently two links available on the SystmOne patient screen to achieve connectivity – one for Milton Keynes Health Information Exchange (HIE) and its providers, and one for the Bedfordshire Care Alliance Shared Health and Care Record (BCA ShCR).
All BLMK GP practices should already be connected to Milton Keynes HIE, and the document How to connect to Share for Care – information for GPs explains what you need to do to sign the Information Sharing Framework and connect to the BCA ShCR.
