The Advanced Kidney Care Clinical Nurse Specialists provide patient education in preparation for renal replacement therapy for patients who have chronic kidney disease stages 4 and 5.
The team carry out home visits, virtual or face-to-face appointments that combine nursing and psychosocial patient assessment with the renal support team and/or transplant coordinators.
The team also monitor blood test results, ensure patients are up to date with hepatitis screening and vaccination and anaemia management is addressed in collaboration with the renal pharmacists.
